Infant Taylan Phenix Smith died October 8, 2016, at ECM Hospital.
A graveside service will be Sunday, October 16, 2016, 2 PM at Elmwood Cemetery in Towncreek.
Elkins East Chapel is assisting the family.
She was the daughter of Wendy Elmas and Jamie Smith.
