Blake Anthony Phillips, age 17, passed away on Saturday, November 13, 2021.
He was born in Columbia, TN, on May 25, 2004, student at Giles County High School, where he was a member of the Giles County High School Band, he was preceded in death by his father, Robert Phillips and grandfather, Aaron Phillips.
Visitation will be on Wednesday, November 17, 2021, from 4PM to 8PM at the Carr & Erwin Funeral Home. Funeral service will be held on Thursday, November 18-2021, at 2PM at the Carr & Erwin Funeral Home with burial to follow in Giles Memory Garden.
Survivors are: grandmother, Emily Phillips, Pulaski, aunt, Jackie Koppelt, Pulaski, God parents, James & Karen May, Goodspring, sister, Clarissa Phillips, Pulaski.
