Survivors include his daughter, Harper Emory Polk; parents, Rick and Sherry Polk; sisters, Paulette Polk-Scanlan (Peter Scanlan) of Belmont, MA and Paige Mastro (Patrick Griffin) of Portsmouth, RI; nieces, Abigail and Caroline Scanlan and Emery and Riley Mastro; and uncles, Reggie Lamons and Philip Polk.
