It is with great sadness that the family of Carolyn Marie Polk announce her passing on Thursday, December 17, 2020 at the age of 51. Carolyn was a medical assistant at St. Vincent’s Primary Care and served as President of the Alabama Society of Medical Assistants. She is survived by her daughters: Caitlyn (Barrett) Eubanks, Chalis (Zac) Joiner, Charlotte Polk and Carson Polk; grandchildren: Jack and Ellie Eubanks; parents: Dennis and Nancy Brooks; brothers: Craig Brooks and Brandon Brooks and grand puppies: Darcy and Titan.
