Phillip Dewayne Lake, 52 of Addison, entered into rest on Wednesday, December 6, 2017 at CRMC.

Phillip was born on March 10, 1965 in Cullman. He loved to fish and hunt and was an avid Alabama fan.

Visitation will be held on Saturday, December 9, 2017 from 11:30 to 1pm at Nichols Funeral Home in Addison, where services will be held at 1pm. Burial will be in Liberty Baptist Church Cemetery. Brother Kenny Sudduth, Jerry Lake and Michael Turner will officiate.

Phillip is survived by his daughter, Kali Lake; mother, Delores Lake; fiancée, Lanette Edmondson; stepson, Abe Rooks and his wife Jennifer; brother, Jason Lake and his wife Katrina; sister, Vickie Smith and her husband Darrell; he was grandpa to 2 grandchildren, Bentley and Cooper Rooks; and niece, Macy Lake.

He was preceded in death by his father, Bobby Lake; sister, Kimberly Lake; grandparents, William and Aree Key, and J.V. and Elease Elam.
